San Juan Basin Health Dental Clinic (formerly known as Southwest Smile Makers) will be providing dental services at Park on February 6th & 8th. All students can participate. Students who received a dental cleaning in the last 6 months may not need this service.

Dental services include:
• Dental screening by a Registered Hygienist
• Dental fluoride application
• Oral hygiene education (i.e. good brushing, etc.)
• Sealants
• Referrals to a dentist (if decay/cavities or other concerns suspected)

Additional Service at low cost or covered if child has CHP+ or CO Medicaid:
• Teeth cleaning: $40 (age 6 & under) or $50 (ages 7 & up)

CHP+, Colorado Medicaid as well as cash/check payments will be accepted. All services are covered by CHP+, Medicaid and the Dental Clinic will bill these insurances for you. If you would like your child to receive these services, consent forms and a letter will be coming home in Thursday folders on Thurs., Jan. 18th. Please return forms to the nurse’s office.
